Risiko und Fehlervorsorge sind wichtige Mittel, die Patientensicherheit zu verbessern. Ein Risiko ist jedes, aus der Sicht eines Berichtenden, zukünftig zu vermeidendes Ereignis. Solche Ereignisse sind häufig die Vorstufe zu Fehlern. Vorsorge ist effektiv, wenn Fehler vermieden werden.
